By creating a user name, you will be able to "re-access" your survey again without losing your data in the event that you do not complete the survey all at once.  The user name can be anything you like, but it must be unique from others who have already completed the survey.  The best way to do this is to use your email address.  If you do provide your email address, it will only be used to:

• Ensure that we do not receive duplicate responses.
• To give you, the respondent, a way to access your survey again if you do not complete it all at once.  By entering your email address, you will be taken to the last page that you completed.
• To send you one (1) reminder email to complete the survey before the deadline.  This reminder is only sent if the survey is not complete.

Your email address will not be used by the Association Resource Centre Inc. for any purpose other than what is stated above.  You will be asked at the end of the survey to provide contact information so that we can provide you with the survey results.
